स्व-घोषणा पत्र, written for you in Hindi
Tehsil offices, ration card counters, schools and scheme camps across Hindi-speaking states often ask for a स्व-घोषणा पत्र with an application. People usually copy a sample by hand or ask a typist, and small mistakes creep in: the wrong verb form for a woman applicant, the income written only in figures, or the closing declaration left out. This page opens the self-declaration tool in Hindi, so you get a complete, correctly worded declaration with very little typing.
What it looks like
A typical Hindi declaration made with this tool has:
- सेवा में and the office's name and address, if you add one, such as तहसीलदार महोदय, तहसील सदर.
- A heading such as आय का स्व-घोषणा पत्र or निवास का स्व-घोषणा पत्र.
- The opening line: मैं, [नाम], पुत्र / पुत्री / पत्नी [नाम], आयु लगभग [आयु] वर्ष, निवासी [पता], एतद्द्वारा घोषणा करता / करती हूँ कि:
- Numbered statements for what you are declaring.
- The closing declaration that the information is true, nothing has been concealed, and that false information can lead to action under the law.
- स्थान, दिनांक, and the signature with your name and mobile number.

Tips for Hindi declarations
- Write your name exactly as on Aadhaar or the ration card. You may type it in English if you don't have a Hindi keyboard; the rest of the declaration stays in Hindi.
- For income, give the financial year (वित्तीय वर्ष), such as 2025-26. The amount is printed in figures and Hindi words, for example ₹96,000 (छियानवे हज़ार रुपये मात्र).
- Choose your relation carefully: पुत्री or पत्नी makes the verbs feminine, and पुत्र or पति makes them masculine.
- Sign in ink on the printed page and write the date. Keep a photocopy.
If the declaration lists documents (दस्तावेज़), self-attest each photocopy by signing it and writing the date, and attach them in the same order. If the counter asks for a photograph, paste a recent passport-size photo at the top. Read the printed Hindi once before signing: a wrong year or a missing family member can get a declaration sent back.

Will my verbs be correct for a woman?
Yes. Choose पुत्री or पत्नी as your relation and the declaration says करती हूँ, रही हूँ and so on. For पुत्र or पति it uses the masculine forms.
Can I use it for a ration card or income certificate?
Use the Income format for an income certificate, and the Other format to write your own statements, for example about a lost ration card. If the department has its own printed declaration, use that instead.
